About The Position

Jericho Road Community Health Center is actively seeking Physician Assistants (PA) and Nurse Practitioners (NP) to join their Medical Care Department. These full-time positions will serve adult, pediatric, and obstetric patients across the Barton, Broadway, & Doat locations. The roles involve providing high-quality, patient and family-centered primary care, ensuring cost-effective services, and maintaining electronic medical records efficiently. Collaboration with interdisciplinary teams, participation in quality improvement activities, and a commitment to a holistic approach to patient care (physical, mental, emotional, and spiritual health) are essential. PAs will also participate in an inpatient call pool and work with residents. NPs will participate in an office call pool for overnight patient phone calls and work one Saturday morning clinic per month. Both roles require adherence to professional ethics and compliance with Article 28 and FQHC requirements, as well as maintaining current certifications and CPR training.

Requirements

  • Doctor of Medicine or Doctor of Osteopathy, licensed to practice in New York State prior to start date of employment (for PA).
  • Completion of three-year or four-year Family Medicine Residency (for PA).
  • Board-eligible from American Board of Family Medicine. Board-certified within one year of employment (for PA).
  • FNP (family nurse practitioner) preferred or ANP (adult nurse practitioner) (for NP).
  • RN, Masters or Nurse Doctorate (ND) Certification by American Nurses Credentialing Counsel (ANCC), American Academy of Nurse Practitioners, or equivalent certifying body (for NP).
  • Board-certified within one year of employment (for NP).

Responsibilities

  • Provide high quality primary care for adult, pediatric, and obstetric patients.
  • Offer comprehensive, patient and family centered care.
  • Ensure consistent, cost-effective care.
  • Maintain electronic medical records efficiently and maximize resources.
  • Collaborate with interdisciplinary clinical and program teams.
  • Create a learning plan that includes familiarity with new research and changes to standard of practice.
  • Participate in quality improvement activities with a data-driven approach.
  • Provide inpatient adult, pediatric, and obstetric care as part of a call pool with other family medicine and OB/GYN physicians (for PA).
  • Willingness to work with family medicine and pediatrics residents (for PA).
  • Attend required staff and provider meetings except when clinical activities prevent such attendance.
  • Provide patient care in a wholistic manner including attention to physical, mental, emotional, and spiritual health.
  • Maintain current certifications including all mandatory annual and CPR training.
  • Ability to adhere to professional code of ethics, and compliance to Article 28 and FQHC requirements.
  • Improve patient outcomes and deliver quality care within practice areas as assigned (for NP).
  • Follow up with patients regarding lab results or other medical care concerns (for NP).
  • Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, state/local regulations, and patient satisfaction (for NP).
  • Participate in an office call pool for patient phone calls overnight (for NP).
  • Work one Saturday morning clinic per month (for NP).
