About The Position

Physician Assistant/Nurse Practitioner Supervisor (NPS) Bronx/Westchester (Primarily Bronx) 3 Days a Week, M-F, 8am to 5pm Temp to Perm FlexStaff is seeking a Physician Assistant/Nurse Practitioner Supervisor (NPS) for our client, a non-profit healthcare organization providing home and community-based healthcare and services for the elderly. PURPOSE: This position will collaborate with community primary care physicians (CPCPs) to oversee and coordinate participant care within the Interdisciplinary Team (IDT). This role focuses on participant independence and wellness, acting as a liaison between the participant, CPCP, and plan staff to develop and implement care plans.

Requirements

  • Graduate of NYSED-acceptable nurse practitioner program or nationally certified.
  • Minimum 2 years of clinical nursing experience in home care, care management, or related field.
  • Minimum 1 year of experience with frail/elderly populations (or training provided).
  • Excellent communication, interpersonal, analytical, and research skills.
  • Project management skills and ability to meet deadlines.
  • Proficient in Microsoft Office.
  • Active NY Nurse Practitioner license.
  • DEA certification.
  • AANP or ANCC Board Certification.
  • Legally authorized to practice in NY.
  • Medically cleared for communicable diseases and up-to-date immunizations.
  • Medicaid and Medicare Provider IDs in good standing.

  • Bilingual preferred.

Responsibilities

  • Supervise and manage Nurse Practitioners and other clinical staff at assigned PACE site.
  • Conduct performance appraisals.
  • Represent/serve as Medical Director in their absence or as directed.
  • Perform annual wellness assessments (more frequently for high-risk participants).
  • Provide urgent care and same-day access.
  • Manage participants with acute changes in condition.
  • Assess participants returning from hospital/ED within 5 days, reconcile medications, and coordinate follow-up.
  • Review medical history, formulate diagnoses, and ensure accurate documentation.
  • Participate in clinical documentation improvement activities and provide feedback to staff.
  • Review symptoms, identify active diagnoses, and inform PCPs of potential care gaps.
  • Co-manage high-risk participants with PCPs and develop care plans to address gaps.
  • Attend IDT meetings and represent CPCP as needed.
  • Communicate with IDT and other plan designees.
  • Educate participants, families, and staff on disease processes, medication adherence, and self-management.
  • Conduct home visits as needed.
  • Supervise care plan formulation and delivery for community PCP participants.
  • Oversee home care services arranged/provided by CHN.
  • Provide emergency coverage for PACE center PCPs.
  • Participate in on-call rotation.
  • Adhere to scope of practice and meet established competencies.
  • Act as an IDT member.
  • Perform other duties as assigned.
