Our Hospitalist team is seeking a Part Time (16 Hour) Physician Assistant. The PA will support physicians, collaborate with consultants, nurses, case managers, social workers, rehab providers, and others to provide high-quality, efficient care for general medical patients and for patients on the Short Stay Unit. This role consists of 13-hour day/evening shifts (7:00 AM – 8:00 AM or 10:00 AM – 11:00 PM), with a requirement to work every sixth weekend and participate in a rotating holiday schedule. The position is based on the NWH main campus at 2014 Washington Street, Newton, MA 02462. The Hospitalist PA will be responsible for the following: Caring for patients on the Short Stay Unit and the inpatient med/surg units Completing initial patient evaluations, inclusive of obtaining a history, completing medication reconciliation, performing an exam, reviewing diagnostic testing, and documenting an H&P as well as entry of admission orders Monitoring patient progress and coordinating treatment plans with supervising attending, consultants, and providers in other disciplines Documenting subsequent encounters through completion of progress notes Discharging the patient from the hospital, including: Performing discharge medication reconciliation Working with nurses, case managers, social workers and others to coordinate a discharge care plan, including primary care and/or subspecialist follow-up to ensure safe care transitions Counselling patients and family members, regarding their illness, medications, nutrition, diagnostic tests, procedures, care plan, and post-discharge care Triaging patients from NWH ED as well from outside hospitals Collaborating with nurses, hospitalists, and other providers to accept patients, as appropriate, from NWH ED, outside hospital EDs, and outpatient clinics Communicating with accepting providers to ensure a smooth transition Cross-covering Non-Housestaff and Short Stay Unit patients during later afternoon and evening hours Participating in Short Stay Unit, hospital medicine and hospital-wide process improvement, quality and safety projects Assisting in quality assurance, quality and process improvement and patient safety initiatives within the Hospital The Physician Assistant (PA) is a licensed provider. The PA is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation. May also perform additional duties, such as precepting a small group of learners.
