About The Position

Our Mobile Primary Care Team is seeking a dedicated and compassionate Physician Assistant with a heart for senior care and a passion for redefining how older adults and their caregivers experience health and well-being. This position blends independent practice and team-based collaboration, supported by a model that values compassion, connection, and clinical excellence. At Greenwell Health and Affiliate, we’re redefining traditional healthcare by bringing medical expertise directly to where seniors live – their homes and communities. The Brigade has grown to serve over hundreds of senior living across multiple states, earning recognition for our commitment to personalized, relationship-based care. As a Physician Assistant, you’ll join a purpose-driven team that delivers continuum-based care – integrating primary, psych, palliative, and home-based services. This team is built on a collaborative culture, where operational, clinical, and peer leaders support your long-term success. They value loyalty and partnership with your pod and other Brigade leadership, fostering stability, mentorship, and growth. The team combines clinical excellence and efficiency with compassion, consistency, and connection, and supports your autonomy through structured onboarding, coaching, and clear playbooks. This role is unique because it offers continuum-based care (clinical excellence) where you deliver high-quality care across home, senior living, and community settings, supported by a multidisciplinary team in palliative, psych, and neurology. You will also gain dementia leadership experience, building deep expertise in dementia-informed care and caregiver support as part of our expanding geri-psych and cognitive health initiative. You will experience autonomy with support, where growth and quality are equally valued – encounter volume, patient retention, and outcomes all matter. Finally, you will be part of a collaborative culture, working alongside caring peers and leaders committed to your professional development and success.
