About The Position

This role involves coordinating patient care for an assigned caseload, including appropriate delegation to and supervision of PTA staff. The position requires completing patient condition change visits and OASIS time points, while accepting accountability for patient satisfaction scores and quality of care outcome measures. The Physical Therapy Team Resource will complete physical assessments, develop patient-specific care plans, and initiate preventive and rehabilitative therapy procedures. This role also involves administering medications and treatments as prescribed, counseling patients and families, and respecting cultural beliefs. Health care instructions will be provided as appropriate, and home health aide care will be supervised. The position requires managing episodes of care, coordinating with primary disciplines, and identifying discharge planning needs. Communication with physicians and other team disciplines, including community providers, is essential. Proficiency in OASIS completion and timely documentation are required. Additional duties include participating in weekend/holiday rotations, ensuring necessary equipment and services are available, and participating in in-service programs and precepting other physical therapy personnel.

Requirements

  • Physical Therapist with current licensure to practice therapy in the state of Massachusetts.
  • Active American Health Association BLS. If no American Heart Association BLS the employee must complete an annual BILHAH BLS Competency. No other forms of BLS will be accepted.
  • Graduate of an accredited program in Physical Therapy.
  • Minimum of 1-2 years physical therapy experience, at least one of which is in the area of public health or home care is preferred.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
  • Self-directed and able to work with minimal supervision.
  • Shows strong ability to organize and prioritize workload independently; physical therapy skills per competency checklist.

Nice To Haves

  • DPT preferred.
  • OASIS experience is preferred.

Responsibilities

  • Coordinate patient care for assigned caseload, including appropriate delegation to PTA staff and supervision of care provided by PTA staff.
  • Complete patient condition change visits and OASIS time points.
  • Accept accountability for patient satisfaction scores and quality of care outcome measure results.
  • Complete and document ante physical assessment and history of current and previous illness(es).
  • Develop a patient specific plan of care, which establishes goals incorporating therapeutic, preventive, and rehabilitative actions.
  • Include the patient and the family in the planning process.
  • Initiate the plan of care and make necessary revisions as patient status and needs change.
  • Initiate appropriate preventive and rehabilitative therapy procedures.
  • Administer medications and treatments as prescribed by the physician.
  • Counsel the patient and family in meeting therapy and related needs.
  • Respect and support patient and family’s cultural beliefs and values, individual needs, health goals and treatment preferences.
  • Provide health care instructions to the patient as appropriate per assessment and plan of care.
  • Instruct, supervise and evaluate home health aide care provided every two (2) weeks as necessary.
  • Help manage episode of care or coordination of care with the primary disciplines in accordance with evolving occupational therapy needs and continued eligibility for services.
  • Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient.
  • Communicate with the physician regarding the patient’s needs and report any changes in the patient’s condition; obtain/receive physician’s orders as required.
  • Attend regular case conferences and team meetings per Clinical Manager and as needed.
  • Communicate with team PTA when delegating appropriate visits.
  • Communicate with other team disciplines and community providers to coordinate the care plan (i.e. insurance case managers, high risk case managers, elder services, protective service, etc).
  • Demonstrate proficiency in OASIS completion.
  • Complete clinical visit notes explaining the need for the skilled physical therapy service in light of the patient’s overall medical condition and experiences; the complexity of the services performed and the plan for the next visit based on the rationale of prior results.
  • Complete documentation in accordance with agency timeliness policies.
  • Participate in the weekend/holiday rotation as defined by Lahey Health at Home.
  • Ensure that arrangements for equipment and other necessary items and services are available.
  • Participate in in-service programs and supervise, teach and precept other physical therapy personnel per Clinical Manager.
