When you join the growing BILH team, you're not just taking a job, you’re making a difference in people’s lives. This role involves coordinating patient care for an assigned caseload, including appropriate delegation to and supervision of PTA staff. The position requires completing patient condition change visits and OASIS time points, while accepting accountability for patient satisfaction scores and quality of care outcome measures. The Physical Therapy Team Resource will complete physical assessments, develop patient-specific care plans, and initiate preventive and rehabilitative therapy procedures. This role also involves administering medications and treatments as prescribed, counseling patients and families, and respecting cultural beliefs. Health care instructions will be provided as appropriate, and home health aide care will be supervised. The position requires managing episodes of care, coordinating with primary disciplines, and identifying discharge planning needs. Communication with physicians and other team disciplines, including community providers, is essential. Proficiency in OASIS completion and timely documentation are required. Additional duties include participating in weekend/holiday rotations, ensuring necessary equipment and services are available, and participating in in-service programs and precepting other physical therapy personnel.
