The Physical Therapist (PT) is a qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients/clients in the home as prescribed by the attending physician.

Requirements

  • Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
  • Licensed or registered to practice as a PT in the state of practice.
  • Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
  • Has the ability to read and follow written instructions and document care given.
  • Is self-directing and has the ability to work with little direct supervision.
  • Possesses and maintains current CPR certification.
  • Is flexible and cooperative in fulfilling the role obligations.
  • Demonstrates tact, patience, and good personal hygiene.
  • Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
  • Must be able to verbally communicate in the language of the client and document all client-related activity practices.
  • Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

Responsibilities

  • Providing treatment as ordered by the attending physician, dentist, or podiatrist.
  • Assisting in developing and updating the plan of treatment or Plan of Care (POC).
  • Assisting the physician in evaluating level of function.
  • Observing, recording, and reporting information on the patient’s condition to the attending physician, case manager, and in the patient’s health record.
  • Advising, consulting, and when appropriate, instructing family and other LORIAN personnel in patient’s therapy program.
  • Teaching and supervising other healthcare personnel when appropriate.
  • Evaluating the home environment and making appropriate recommendations.
  • Completes, maintains, and submits accurate and relevant clinical notes regarding patients/clients condition and care given in accordance with LORIAN policies and procedures.
