About The Position

This position requires a licensed Physical Therapist with at least one year of experience in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) setting, or equivalent rehabilitation experience. The role involves assessing patients, developing treatment plans, and supervising other members of the rehabilitation team. Strong communication and collaboration skills are essential for working with geriatric and rehabilitation populations, as well as interdisciplinary teams.

Requirements

• Minimum 1 year of Physical Therapy experience required.
• LTC/SNF or Rehab experience required.
• Active Physical Therapist License required.
• BLS Certification required.
• Strong patient assessment and treatment planning skills required.
• Ability to supervise PTAs, rehabilitation aides, and students required.
• Excellent communication, documentation, and interdisciplinary collaboration skills required.

Nice To Haves
