About The Position

We are seeking a Physical Therapy Assistant (PTA) for a position in Tulsa, OK. New graduates are welcome to apply. SNF experience is preferred. Candidates must complete and attach a prescreen form, resume, and license verification upon submission. Failure to do so will result in the candidate being declined. A COVID test must be administered 72 hours prior to the start date, with results issued 48 hours prior to the start date.

Requirements

• Valid Oklahoma State Physical Therapy Assistant License.
• Completion of prescreen form.
• Submission of resume.
• Submission of license verification.
• Three references.
• County criminal background check (7 years).
• Federal criminal background check (7 years or more).
• Multi-state criminal search (7 years or more).
• Nationwide sex offender search (7 years or more).
• HHS/OIG search.
• GSA search.
• Social Security Number trace.
• Oklahoma State Background with Fingerprints.
• 1 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray.
• COVID test results issued 48 hours prior to start date.

Nice To Haves

• Skilled Nursing Facility (SNF) experience preferred.
• New graduates are acceptable.

Responsibilities

• Provide physical therapy services as a Physical Therapy Assistant.
• Adhere to credentialing checklist requirements prior to submission, including state license verification, resume, references, county criminal checks (7 years), federal criminal checks (7 years or more), multi-state criminal search (7 years or more), nationwide sex offender search (7 years or more), HHS/OIG search, GSA search, Social Security Number trace, Oklahoma State Background with Fingerprints, and a 1 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray.

• Undergo COVID testing 72 hours prior to start date with results issued 48 hours prior to start date.
