About The Position

This role involves assisting physical therapists in providing comprehensive physical therapy treatments and procedures. The Physical Therapy Assistant will work under the guidance of a physical therapist to develop and implement treatment plans, document patient progress, and modify treatments as needed. Key responsibilities include providing direct patient care to relieve pain and restore function, educating patients and their families on exercises and daily living activities, and maintaining accurate clinical records. The position also requires participation in team meetings and collaboration with other healthcare professionals. A strong understanding of fiscal and regulatory issues related to home health and hospice services is necessary, along with adherence to the company's mission, vision, and core values.

Requirements

  • Possesses knowledge of fiscal and regulatory issues related to home health/ hospice services, specifically physical therapy.
  • Possesses thorough knowledge of and maintains strict adherence to the company’s Mission, Vision, and Core Values.
  • Legally authorized to work in the United States.

Responsibilities

  • Assists physical therapists in providing physical therapy treatments and procedures.
  • Assists in the development of treatment plans, carries out routine functions, documents the progress of treatment and modifies specific treatments in accordance with patient status and within the scope of treatment plans established by a physical therapist.
  • Renders treatment and instructional services to patients/caregivers only within the scope of instructions developed by the Physical Therapist/Physician as prescribed in the plan of care, within scope of LPTA’s practice.
  • Observes, records, and reports to the Physical Therapist and other team members for the purpose of planning, modifying and coordinating treatment programs.
  • Treats patients to relieve pain and/or develop or restore function to reach the highest level of independence possible.
  • Writes clinical progress notes on the date of the visit for inclusion into the patient’s permanent record.
  • Attends Rehab staff meetings, in-service presentations and case conferences.
  • Communicates regularly with the case manager and other health care professionals assigned to the case.
  • Instructs patients and their families in transfer techniques, home exercise program and activities of daily living as assigned by the physical therapist.
  • Completes necessary documentation on a timely basis and in line with procedures.
  • Performs other duties as assigned by management.
