Per Diem Physical Therapy Assistant (PTA)

Requirements
- Current, valid Minnesota PTA license.
- CPR/BLS certification.
- Ability to travel throughout the Twin Cities Metro area.
- Reliable transportation and willingness to travel between patient visits.
- Strong communication, organization, and clinical skills.
- A genuine desire to help patients live more independently.
Nice To Haves
- Previous home health and/or outpatient experience strongly preferred.
Responsibilities
- Provide skilled physical therapy services under the direction of a supervising Physical Therapist.
- Implement individualized treatment plans focused on mobility, strength, balance, and functional independence.
- Monitor patient progress and communicate changes in condition to the supervising PT and care team.
- Educate patients and caregivers on home exercise programs, safe mobility, fall prevention, and appropriate use of assistive devices.
- Complete timely and accurate clinical documentation.
- Collaborate with the interdisciplinary care team to support positive patient outcomes.
