Requirements
- Accredited PTA Program graduate with an active California PTA license.
- Current CPR/BLS certification.
- Ability to work independently, travel locally, and adapt to changing participant needs.
- Strong communication, documentation, and teamwork skills.
Nice To Haves
- Experience working with geriatric populations or in long-term care, home health, or PACE preferred.
Responsibilities
- Implement individualized treatment plans including therapeutic exercise, gait training, balance activities, and functional mobility tasks under the supervision of the PT.
- Observe, document, and report participant progress, functional changes, and safety concerns to the supervising PT.
- Coordinate with nurses, social workers, occupational therapists, and other PACE team members to ensure integrated, person-centered care.
- Provide therapy services in participant homes, the PACE day center, and clinic environments as needed.
- Train participants and caregivers on safe use of assistive devices and mobility equipment; maintain a safe therapeutic environment.
- Complete accurate, timely electronic health record (EHR) documentation in compliance with PACE, state, and federal guidelines.
