About The Position

Arbor Home Health Therapy provides staffing for Physical Therapy, Occupational Therapy and Speech Therapy to home health agencies. We are seeking a Physical Therapy Assistant to provide physical therapy services in the community and Assisted Living settings. The ideal candidate will be self-motivated, organized, and responsible for patient care, including scheduling appointments and timely documentation. This role involves entering patient homes in the community and Assisted Living facilities, and committing to following the evaluating therapist's Plan of Care (POC) for routine follow-up visits. The pay rate is a flat, per-visit rate of $35. Reliable transportation and willingness to travel within a 45-mile radius are expected.

Requirements

  • Valid Physical Therapy Assistant License
  • CPR Certification
  • Excellent observation, verbal and written communication skills
  • Critical thinking skills
  • Basic math skills
  • Compassion, patience, and a positive attitude
  • Reliable transportation
  • Personal Home Health Kit including thermometer, blood pressure cuff & stethoscope (automated device acceptable), and pulse oximizer.

Nice To Haves

  • Experience working with patients in their own homes.
  • Experience in Home Health settings.
  • Experience with documentation systems such as Therapy Boss, Kinnser, or Home Care Home Base.

Responsibilities

  • Provide physical therapy services in the community and Assisted Living settings.
  • Schedule patient appointments.
  • Complete documentation of visits within 48 hours.
  • Communicate with the Clinical Program Manager and therapy team.
  • Follow the evaluating therapist's POC for routine follow-up visits.
  • Travel within a 45-mile radius from home.
  • Demonstrate a helpful, positive attitude with all patients and caregivers.
  • Manage time for each visit.
  • Report missed visits and changes in patient condition to the Home Health Clinical Program Manager.
  • Review documentation systems daily to correct/resubmit outstanding documentation.
  • Maintain current licensure in good standing.
