About The Position

Bring your expertise and skills as a member of the team, help manage the complex medical, functional, and social needs of older adults in our Northern Kentucky PACE program. Ultimately, PACE will work to maximize the dignity and unique needs of seniors and their caregivers. Senior CommUnity Care of Northern KY PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare affiliate of the Volunteers of America parent organization.

Requirements

  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver’s license and have means of transportation, to assist participants in their homes or other settings.
  • Must clear the background check.
  • Graduate of a Physical Therapist Assistant curriculum, with a active PTA license in Kentucky.
  • Minimum of one-year experience working with a frail or elderly population required.
  • Good working knowledge of physical, psychosocial and behavioral needs of the older population.
  • Effective listening, and written and oral communication skills.
  • Demonstrated ability to problem solve and apply good judgment.
  • Demonstrated ability to efficiently and effectively manage multiple priorities.
  • Working knowledge of practice boundaries for Physical Therapist and Physical Therapy Assistant (P.T.A.).
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
  • Only acts within the scope of his or her authority to practice. Must meet a standardized set of competencies for the specific position description established by Senior CommUnity Care and in accordance with CMS before working independently.

Responsibilities

  • Perform physical therapy treatments and related duties as may be delegated by the physical therapist.
  • Communicate participant’s changes and progress per treatment plan on a regular basis to Registered Physical Therapist.
  • Maintain proper documentation according to state policies and, federal PACE regulations.
  • Maintain proper records according to PACE policy and procedures in the EMR and as otherwise required.
  • Perform and record, if required, as specified by the planned treatment program, non-evaluative on going data.
  • Perform other duties as assigned.
