About The Position

We are seeking cheerful and dynamic per diem Physical Therapy and Occupational Therapy Assistants to join our skilled team at Forest View Center for Rehabilitation and Nursing. You will play a crucial role in providing comprehensive rehabilitative care to our residents. Working under the guidance and supervision of licensed OT/PTs, you will assist in implementing personalized treatment plans, monitoring progress, and helping individuals regain optimal physical function and independence.

Requirements

• Completion of an accredited OTA/PTA Program.
• Current and valid license or certification as a OTA/PTA in NY.
• Prior experience in a skilled nursing facility or geriatric care setting is required.
• Strong understanding of therapy principles, practices, and techniques.
• Knowledge of commonly used therapeutic modalities, equipment, and assistive devices.
• Strong interpersonal skills to communicate effectively with residents, families, and interdisciplinary team members.
• Proficient computer skills for documentation and record keeping. (Rehab Optima (Net health) and Sigma Care).
• Compassion, patience, and a genuine desire to improve the quality of life for residents.

Responsibilities

• Collaborate with licensed OT/PT to evaluate residents' physical abilities, strengths, and limitations to develop individualized treatment plans.
• Assist in implementing therapeutic interventions, exercises, and modalities to improve residents' mobility, strength, balance, coordination, ambulation, and functional independence.
• Guide and instruct residents in performing prescribed therapeutic exercises and activities to ensure proper technique and safety.
• Monitor residents' progress, record observations, and communicate findings to the supervisor and interdisciplinary team.
• Utilize therapeutic equipment and assistive devices effectively and safely during treatment sessions.
• Provide education and guidance to residents, families, and caregivers on therapeutic techniques, exercises, and home exercise programs to support continuity of care.
• Maintain accurate and timely documentation of treatment sessions, progress notes, and other required records in compliance with regulatory standards.
• Adhere to ethical and professional standards as set forth by relevant regulatory bodies and professional associations (HIPAA compliance).
