About The Position
Eddy Visiting Nurse & Rehab Association is seeking Full-Time Homecare Physical Therapists and Occupational Therapists. This role involves managing a caseload of patients requiring home-based therapy, primarily post-acute or subacute. The position offers independence, with the ability to start and end your day from home after training. Therapists will utilize cutting-edge technology to enhance patient care and will be part of a supportive team. The agency is recognized for its high quality of care, boasting a 5-star rating. New hires will receive thorough orientation and mentorship.
Requirements
- Current NYS PT or OT license and registration
- Minimum of one (1) year of experience as a PT or OT, preferably inpatient or homecare, is required
- Valid Driver's License and Reliable Vehicle.
- Home care experience preferred, but not required

Responsibilities
- Promote/provide physical therapy and/or occupational therapy services in the home to assist the patient in achieving his/her optimal level of functioning.
- Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.
- Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es).
- Coordinate an interdisciplinary plan of care with nursing/physical/occupational/speech therapists, medical social workers, aides and dieticians.
