About The Position
UW Health in northern Illinois is seeking a full-time Physical Therapist for their Home Health program. This role involves providing outpatient physical therapy services. The position is 100% FTE, working a day shift schedule from Monday to Friday, 8:00 a.m. to 4:30 p.m., with every third Saturday also from 8:00 a.m. to 4:30 p.m. The work location is the UW Health 2550 Charles St Clinic.
Requirements
- Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation).
- Current license from the State of Illinois.
- Basic Life Support/CPR Certification.
- Valid State driver’s license and proof of liability insurance.
- Must present acceptable evidence of licensure or authorization to test at the time of employment. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided.
