Dove Healthcare consists of 11 skilled nursing and rehabilitation centers, six assisted living residences, and three independent living complexes across various locations in Wisconsin. Their core services include inpatient and outpatient rehabilitation, post-hospital care, ventilator care, skilled nursing, long-term and end-of-life care, as well as senior independent living, assisted living, and memory care. Requirements
- current license as a physical therapist in the State of Wisconsin
Responsibilities
- plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
- participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
- maintain treatment records, patient files, and progress notes as required
- participate in the development and implementation of patient assessments and care plans, including quarterly reviews
