Home Health Physical Therapist- Calumet City
About The Position
Looking for a licensed Physical Therapist for part time full time or per diem work to visit patients in their home and provide home exercise program, safety evaluation, and promote and prevent falls and improve strengthening, ROM and muscle education. We receive referrals in the following areas: Calumet City and surrounding south Subs. If you enjoy working with one patient at a time and improving their overall health, safety and mobility this role is for you. We are looking for a dynamic and motivational therapist who sets goals for the patient/family and works with our interdisciplinary team to achieve clinical outcomes.
Requirements
- Licensed Physical Therapist
- Dynamic and motivational therapist
Responsibilities
- Visit patients in their home
- Provide home exercise program
- Conduct safety evaluations
- Promote fall prevention
- Improve patient strengthening
- Improve patient range of motion (ROM)
- Provide muscle education
- Set goals for the patient/family
- Work with an interdisciplinary team to achieve clinical outcomes
