About The Position

The Rehabilitation Services Staff Physical Therapist provides evaluation and treatment of patients in a range of specialty programs. This role involves maintaining good rapport and cooperative relationships, ensuring patient confidentiality, and behaving in accordance with the Code of Conduct, Service Excellence Standards, and the Mission, Vision, Values and policies of Chelsea Hospital. The therapist is responsible for performing job duties in the safest possible manner, demonstrating self-directed learning, and providing quality patient care by considering age-specific, developmental, and cultural needs. They must also demonstrate unit/area competencies, promote a positive work environment through teamwork, and provide individualized assessment of patients. The role includes creating appropriate plans of care, staying up-to-date on insurance guidelines, providing thorough and timely documentation, formulating individualized therapy goals, consulting with team members, and implementing patient care consistent with goals and patient needs. Additionally, the therapist will complete daily charges, orient patients and families, provide daily oversight of physical therapy provided by staff working under them, conduct re-evaluations, supervise student affiliates, and contribute to their own professional growth and staff development. They will also work with management to ensure schedules are managed well for productivity and budget adherence. Acute Care physical therapists on the IP Rehab Unit will participate in interdisciplinary team meetings. The position also includes performing other duties as assigned.

Requirements

  • Bachelors Degree or higher in Physical Therapy from accredited university
  • Licensed as a Physical Therapist in the State of Michigan
  • CPR/ BLS Certification

  • Provides a thorough physical therapy evaluation and creates an appropriate plan of care based on the patient deficits.
  • Stays up to date on all participating insurance guidelines for rehabilitation services.
  • Provides thorough documentation on each patient interaction.
  • Formulates individualized, age specific, therapy goals based upon assessment results and patient needs.
  • Consults with other team members and selects the most appropriate plan of care consistent with the needs and capabilities of the patient.
  • Implements patient care in a manner consistent with the therapy goals and the changing needs of the patient, using a variety of therapeutic approaches.
  • Completes daily charges for each patient while adhering to ethical practices per insurance guidelines for each payer.
  • Orients patients and families to the therapeutic purposes and procedures of physical therapy, and educates them about home programs, impairments and progress.
  • Provide daily oversight of the physical therapy provided by staff working under the physical therapist (could be physical therapist assistant, athletic trainer or physical therapy aide) and co-sign documentation when required.
  • Provides re-evaluations/re-assessments of patients on a regular basis to ensure progression towards goals.
  • Supervises Physical Therapy student affiliates as assigned by the Manager or Student Program Coordinator.
  • Provides for own professional growth and education and contributes to staff development activities.
  • Works with Manager and secretarial staff to ensure schedules are managed well to achieve targeted productivity and clinic budget.
  • Acute Care physical therapists that see patients on the IP Rehab Unit will be required to participate in interdisciplinary team meetings 1-2 times per week.
  • Participates in regularly scheduled staff meetings and inservices.
  • Performs other duties as assigned.
