Requirements

Graduate of an accredited Physical Therapy program
Active New Hampshire Physical Therapist license
Minimum of one year of PT experience
Current CPR certification
Knowledge of state and federal home health regulations
Excellent clinical judgment, communication, and patient education skills
Compassionate, organized, and committed to delivering exceptional patient care

Nice To Haves

home health experience preferred

Responsibilities

Evaluating patients and developing individualized treatment plans
Restoring strength, mobility, balance, and functional independence
Providing hands-on therapy and therapeutic exercise
Monitoring progress and adjusting care plans as needed
Collaborating with nurses, occupational therapists, speech therapists, home health aides, and other clinicians to coordinate care
Educating patients and family members on exercises, safety strategies, and ongoing recovery
Completing accurate and timely clinical documentation
