About The Position

Requirements

• Doctor of Physical Therapy (DPT) or equivalent from an accredited program
• Active Tennessee Physical Therapist license
• Current CPR certification
• Valid driver's license, reliable transportation, and current automobile insurance
• Strong clinical assessment and communication skills
• Ability to work independently and manage time effectively

Nice To Haves

• Previous home health experience preferred, but not required

Responsibilities

• Perform in-home evaluations to assess patient needs and develop individualized treatment plans
• Provide skilled physical therapy services to improve mobility, strength, balance, and overall function
• Monitor patient progress and modify treatment plans as appropriate
• Educate patients and caregivers on home exercise programs, safety measures, and injury prevention
• Complete accurate and timely clinical documentation in accordance with company policies and state regulations
• Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure coordinated patient care
• Travel to patient homes within the assigned service area
