Physical Therapist (PT) - Home Health Case Manager, Chicago South Side
About The Position
As a Home Health Physical Therapist, you’ll have the opportunity to manage your own caseload while being supported by a highly collaborative, integrated care team. This role offers autonomy, meaningful patient relationships, and the ability to make a direct impact on patient independence and quality of life in the home setting. You will provide comprehensive physical therapy assessments and develop individualized care plans. Deliver in-home therapy focused on improving mobility, safety, and independence. Serve as a case manager, coordinating care with physicians and interdisciplinary teams. Educate patients and families to support recovery and long-term outcomes. Travel throughout the assigned North Side Chicago territory.
Requirements
- Licensed Physical Therapist in Illinois (or eligible)
- Minimum 1 year of clinical experience (or strong clinical affiliation experience)
- Experience with home-based therapy equipment and assistive devices
- Strong communication, documentation, and time management skills
- Ability to work independently while collaborating with a broader care team
