About The Position

We are seeking a dedicated Physical Therapist (PT) to join our team in a Skilled Nursing Facility (SNF) setting. This role focuses on providing post-hospital and short-term rehabilitation services to patients, helping them regain mobility and functional independence. The ideal candidate will possess strong clinical skills, excellent communication, and the ability to manage a diverse patient caseload independently.

Requirements

• Active Vermont Physical Therapist (PT) license required.
• Minimum 2 years of recent Physical Therapy experience required.
• Current BLS certification required.
• Strong knowledge of therapeutic exercise, gait training, balance training, mobility rehabilitation, and functional recovery.
• Strong clinical documentation, communication, and interdisciplinary collaboration skills.
• Ability to work independently while managing a diverse patient caseload.

Nice To Haves

• Skilled Nursing Facility (SNF) and/or rehabilitation experience preferred.
• Experience providing post-hospital and short-term rehabilitation services preferred.
