About The Position

Human Touch Home Health is seeking a Full-Time Physical Therapist (PT) to join our growing Fredericksburg-area therapy team. Patient opportunities may include Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, Orange, Locust Grove, Bowling Green, Colonial Beach, and rural surrounding communities. We work to create manageable patient assignments and routes based on location and service needs. As a home health PT, you're not practicing mobility in a controlled environment. You're helping patients navigate their actual stairs, get out of their own beds and chairs, reduce their fall risk, and regain the confidence to move through everyday life. Requirements

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical assessment and communication skills
• Ability to independently manage a home health caseload

Nice To Haves

• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus

Responsibilities

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength, endurance, and functional independence
• Assess home safety and identify fall risks
• Educate patients and caregivers on mobility, safety, and home exercise programs
• Monitor patient progress and adjust treatment plans as needed
• Complete reassessments and discharge planning
• Coordinate care with physicians, nurses, therapists, and other team members
• Complete accurate and timely documentation using WellSky
