About The Position
We are seeking a Part-Time Physical Therapist (PT) to join our skilled nursing facility team. This is a great opportunity to work alongside an experienced interdisciplinary team dedicated to helping residents improve mobility, maximize function, and enhance quality of life.
Requirements
- Active PT license
Responsibilities
- Evaluate residents and create individualized treatment plans
- Provide therapy focused on strength, balance, mobility, and functional independence
- Collaborate with PTA staff and the interdisciplinary care team
- Monitor resident progress and modify treatment plans as appropriate
- Complete accurate and timely documentation
