About The Position

Elderwood Skilled Nursing & Rehabilitation is seeking a compassionate and motivated Physical Therapist (PT) to join their rehabilitation team within their skilled nursing and rehabilitation communities. Requirements

Graduation from an approved school of physical therapy required.
State license to practice physical therapy required.
Ability to direct others necessary.
Must be able to read, write, speak, and understand the English language at an intermediate or more advanced level.

Responsibilities

Provide skilled physical therapy evaluations and treatments in accordance with physician orders
Develop and update individualized care plans and therapy goals
Maintain timely and accurate documentation
Collaborate with interdisciplinary clinical teams regarding resident progress and needs
Participate in resident care planning meetings
Assist with education and training initiatives as needed
Support a safe, respectful, resident-centered care environment
