About The Position

Human Touch Home Health is seeking a PRN Physical Therapist (PT) to serve patients in Warrenton, Culpeper, and surrounding communities. This opportunity offers flexibility without requiring coverage of a massive territory, allowing the therapist to choose service areas and build a caseload around their availability. The role focuses on providing one-on-one patient care in patients' homes to help them improve mobility and functional independence.

Requirements

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical assessment and communication skills
• Ability to work independently in patients' homes

Nice To Haves

• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus

Responsibilities

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Help patients improve strength and functional independence
• Assess home safety and fall risks
• Educate patients and caregivers
• Monitor progress and adjust treatment plans as needed
• Coordinate care with physicians, nurses, therapists, and other team members
• Complete accurate and timely documentation using WellSky
• Participate in reassessments and discharge planning
