About The Position

Tap Healthcare Solutions is seeking a compassionate and motivated Physical Therapist (PT) to join WelbeHealth in Modesto, California. This opportunity is ideal for a therapist who enjoys working with adults and older adults and wants to use their clinical expertise to help individuals maintain mobility, strength, independence, and a higher quality of life. In this role, you will assess participants' mobility and functional needs, develop individualized treatment plans, and provide therapeutic interventions tailored to an adult and geriatric population. You'll collaborate with an interdisciplinary care team to address complex needs and help participants remain as active and independent as possible. This full-time opportunity offers a competitive salary of $112,000 – $136,000 annually and the chance to make a meaningful, long-term difference in the lives of the people you serve.

Requirements

  • Bachelor's degree in Physical Therapy from an accredited program.
  • Valid California Physical Therapist (PT) license or documented proof of eligibility for California licensure.
  • One (1) to two (2) years of professional experience.
  • Ability to obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter.
  • Strong clinical assessment, treatment planning, and patient education skills.
  • Ability to provide individualized physical therapy services to adults with varying functional and healthcare needs.
  • Strong communication and interdisciplinary collaboration skills.
  • Willingness and ability to travel occasionally as required.

Nice To Haves

  • Master's or Doctoral degree in Physical Therapy.
  • CPR and First Aid certification upon hire.
  • Professional experience working with adult and/or geriatric populations.
  • Experience treating older adults with mobility limitations, chronic conditions, or complex healthcare needs.
  • Experience developing individualized rehabilitation programs focused on mobility, strength, balance, fall prevention, and functional independence.
