About The Position

McLeod Health Cheraw, in partnership with Lifepoint Rehabilitation, is seeking a Physical Therapist to join its growing outpatient rehabilitation team. Requirements

• Graduate of an accredited Physical Therapy program
• Current and unrestricted Physical Therapy license in South Carolina
• Current CPR certification
• Strong organizational and communication skills

Nice To Haves

• Previous outpatient or rehabilitation experience preferred, but not required

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Communicate patient progress and collaborate with interdisciplinary team members
• Assist with patient scheduling and documentation
• Maintain accurate patient records in accordance with regulatory and accreditation standards
• Educate patients, families, and nursing staff on follow-through care programs
• Maintain equipment and ensure a safe, clean treatment environment
• Patient evaluations and functional assessments
• Development and implementation of individualized treatment plans
• Ongoing patient monitoring and progress documentation
• Family and caregiver education
• Collaboration with physicians, nursing staff, and rehabilitation team members
