About The Position

Full-Time Physical Therapist Are you a compassionate and driven Physical Therapist who is passionate about helping patients achieve their highest level of independence and mobility? Join our team and play a vital role in restoring function, reducing pain, improving mobility, and enhancing the quality of life for our residents and patients. As a Full-Time Physical Therapist, you will develop individualized treatment plans, collaborate with interdisciplinary teams, and guide patients through their rehabilitation journey while ensuring compliance with all state and federal regulations.

Requirements

  • Bachelor’s, Master’s or Doctorate Degree from a college or university with an accredited Physical Therapy program.
  • Current Physical Therapist license in the state employed.
  • Current CPR certification.

Nice To Haves

  • Six months experience in a long-term care environment preferred.

Responsibilities

  • Evaluate patients' physical therapy needs and develop individualized treatment plans based on physician orders.
  • Establish therapy goals tailored to each patient's abilities, needs, and tolerance levels.
  • Implement, evaluate, and modify treatment interventions to maximize outcomes.
  • Monitor patient progress and adjust care plans to help patients achieve realistic and measurable goals.
  • Provide screenings and therapeutic services designed to improve function, mobility, strength, and overall quality of life.
  • Work closely with physicians, nurses, therapy staff, and other healthcare professionals to coordinate patient care.
  • Participate in interdisciplinary care planning and contribute to team goals.
  • Communicate patient status, progress, and treatment recommendations to patients, families, and care teams.
  • Educate patients and family members on therapy techniques, treatment plans, and ongoing care needs.
  • Maintain accurate and timely clinical documentation in accordance with state and federal regulations.
  • Ensure therapy services comply with HIPAA requirements and skilled nursing facility guidelines.
  • Adhere to Minimum Data Set (MDS) procedures and support effective care planning processes.
  • Provide clinical guidance, consultation, and supervision to Physical Therapist Assistants (PTAs).
  • Promote best practices in therapy delivery and patient care.
  • Perform additional duties as assigned to support the rehabilitation and healthcare team.
