About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Soquel and the surrounding areas. As an independent contractor, you are your own boss and control your own schedule. You will have the flexibility to see patients when and where you want, with no minimum requirement. FeldCare Connects provides administrative support, including assigning, communication, scheduling, care coordination, and quality assurance. Clinicians have the opportunity for one-on-one treatment, observing, recording, and reporting patient responses to treatment and changes in condition. The Electronic Medical Record system is intuitive, making documentation easy, with many clinicians completing documentation during the visit. The company offers a streamlined referral process through their app, Cliniconnects, allowing contractors to accept or decline patients quickly. FeldCare Connects connects over 2,000 clinicians to patients in their community by contracting with over 900 home health agencies throughout CA, AZ, TX, and FL.

Requirements

• Ability and enthusiasm in working with all ages
• Current Physical Therapist license issued by the state board of Physical Therapy
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapy program
• Reliable personal transportation
• Smartphone

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care.
• Complete documentation using the Electronic Medical Record system.
