About The Position
The Physical Therapist will provide comprehensive physical therapy services to a diverse patient population, focusing on but not limited to medically compromised patients such as those with CHF, COPD, DM, Cardiac conditions, as well as orthopedic, neurological, and general weakness issues.
Requirements
- Current SD licensure as a Physical Therapist.
- Bachelor's degree in Physical Therapy.
- Basic Life Support (BLS) certification.
- Minimum of 1 year of experience in physical therapy.
- General physical therapy skills and medical assessment abilities.
Responsibilities
- Provide physical therapy evaluation and treatment planning.
- Develop and implement care plans and interventions.
- Collaborate with multidisciplinary teams within the Home Health Department.
- Document patient progress accurately using the EPIC charting system.
