About The Position

Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, is seeking Physical Therapists to join their award-winning team. The company has been delivering high-quality, patient-centered care for over two decades and focuses on compassionate healthcare delivered at home. This role involves performing patient assessments, developing treatment plans, collaborating with care teams, and providing education to patients and families. The Physical Therapist will also maintain clinical records, supervise assistants, and ensure coordinated care with healthcare team members. Identifying patient needs for additional services and making referrals are also key responsibilities, along with confirming daily visit schedules and documenting all visits and progress notes on the same day of service.

Requirements

  • Must have a current PT license (state-specific).
  • Graduate of an accredited physical therapy program.
  • Valid BLS certification.
  • Minimum one year of PT experience (home health experience preferred).
  • Excellent communication, organizational, and time management skills.
  • Must have a valid driver’s license and access to a reliable and insured vehicle.
  • Ability to visit homes that may not be wheelchair accessible.
  • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
  • Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
  • Proficiency with electronic medical records is a plus.

Responsibilities

  • Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
  • Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
  • Collaborate with the interdisciplinary care team to optimize patient outcomes.
  • Provide education and support to patients and families/caregivers on therapy programs.
  • Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
  • Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
  • Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
  • Identify patient and family/caregiver needs for additional services and make appropriate referrals.
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Communicate patient progress and changes with physicians and care teams.
  • Document all visits and progress notes on the same day of service.
