About The Position

At Elara Caring, the Physical Therapist helps deliver home care by providing compassionate, individualized care in patients’ homes. The role evaluates patients, develops and adjusts treatment plans, monitors progress, collaborates with the interdisciplinary care team, and maintains accurate clinical documentation. The Physical Therapist may also supervise therapy assistants and support staff, as appropriate.

Requirements

• Graduate of an American Physical Therapy Association (APTA)-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• At least one year of experience in a healthcare facility.
• Valid driver’s license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff, as appropriate.
