About The Position

A physical therapist (PT) working in an inpatient acute rehabilitation unit has a crucial role in helping patients recover from serious injuries, surgeries, or illnesses (e.g., stroke, spinal cord injury, major orthopedic surgery, or neurological conditions). Their responsibilities focus on restoring mobility, strength, and function to help patients transition to a lower level of care (e.g., home or outpatient therapy). The PT utilizes GG scoring to assist patient progress and document functional outcomes in compliance with CMS (Centers for Medicare & Medicaid Services) and other regulatory requirements.

Requirements

  • Current state PT license (or eligible for licensure).
  • Doctorate or Master’s degree in Physical Therapy from an accredited program.
  • 1+ years of PT experience (preferred in inpatient rehab, acute care, or neuro rehab).
  • New grads with strong clinical rotations in rehab settings may be considered.
  • BLS (Basic Life Support) required.
  • Strong knowledge of neurological & orthopedic rehab.
  • Ability to manage medically complex patients (e.g., post-stroke, spinal cord injury, amputees).
  • Excellent communication and teamwork skills.
  • Proficient in EMR documentation.
  • Knowledge of IRF0PAI & CMS regulations for Acute Inpatient Rehab.

Nice To Haves

  • NDT, LSVT BIG, or other specialty certs a plus.

Responsibilities

  • Perform initial and ongoing assessments of patients’ functional mobility, strength, balance, and endurance.
  • Develop personalized treatment plans with measurable goals in collaboration with physicians, OTs, SLPs, social workers/case management and nursing staff.
  • Utilize evidence-based interventions to maximize recovery.
  • Provide intensive therapy to improve: Bed mobility (rolling, sitting edge-of-bed), Transfers (bed-to-chair, toilet, car), Gait training (walking with assistive devices as needed), Stair negotiation, Wheelchair mobility & safety (if applicable).
  • Implement therapeutic exercises for strength, flexibility, and endurance.
  • Use modalities (e.g., electrical stimulation, ultrasound, heat/cold therapy) as appropriate.
  • Educate patients and families on home safety, fall prevention, and proper use of assistive devices.
  • Train caregivers in safe transfer techniques and mobility assistance.
  • Provide guidance on post-discharge exercises and adaptive strategies.
  • Participate in weekly team meetings to discuss patient progress and discharge planning.
  • Communicate effectively with physicians, nurses, case managers, and other therapists.
  • Adjust treatment plans based on patient response and medical status.
  • Accurately complete Section GG functional scoring at admission, discharge, and per IRF-PAI guidelines.
  • Maintain accurate and timely documentation in electronic medical records (EMR).
  • Ensure compliance with Medicare, insurance, and facility guidelines.
  • Complete progress notes, discharge summaries, and equipment justification reports.
  • Follow hospital protocols (e.g. isolation precautions, code responses, etc).
  • Assess patient readiness for discharge and recommend appropriate DME (durable medical equipment).
  • Coordinate with case managers for home health services or outpatient follow-up.
  • Ensure a smooth transition to the next level of care.
