About The Position

BrightStar Care of Acton, Andover, and Lowell is seeking a Full-Time Physical Therapist (PT/RPT) to join their home care team. This role offers consistent full-time hours (40 per week), competitive compensation, comprehensive benefits, performance-based bonuses, and the chance to provide one-on-one care in patients' homes. The Physical Therapist will serve patients in Lowell, Chelmsford, Framingham, Billerica, and surrounding areas along the I-495 belt, collaborating with skilled nursing and interdisciplinary care teams.

Requirements

Graduate of an accredited Physical Therapy program
Current Physical Therapist (PT/RPT) license in Massachusetts
Strong communication, interpersonal, and clinical documentation skills
Ability to work independently and manage a full-time caseload
Reliable transportation and ability to travel within assigned service areas

Nice To Haves

Prior experience in home care of 2 years, skilled nursing, or sub-acute rehab preferred but not required

Responsibilities

Provide comprehensive and compassionate physical therapy services in patients’ homes in accordance with physician orders
Perform initial evaluations and ongoing assessments of patients’ functional abilities
Develop and implement individualized treatment plans to improve mobility, strength, balance, and quality of life
Deliver skilled therapeutic interventions, exercises, and patient/caregiver education
Document patient progress accurately and in a timely manner per company and regulatory standards
Collaborate with nurses, caregivers, physicians, and interdisciplinary teams to ensure coordinated care
Maintain a safe, supportive, and professional therapy environment at all times
