Requirements

• Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master’s degree in Physical Therapy; OR
• Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy; OR
• Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• Qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent.
• Valid driver’s license or reliable, independent transportation to and from patients’ residences.

Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for the older adult population, which may include a patient’s private residence or a patient’s room in an independent living facility.
• Evaluate and treat patients in their home environment to maximize functional outcomes.
• Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation.
• Identify the need for and make referrals to other disciplines to address comprehensive patient needs.
• Implement individualized therapy programs designed to restore, reinforce, and enhance performance.
• Instruct, educate, and train patients and caregivers in skills and functions essential for promoting independence and productivity.
• Direct activities to promote and maintain health.
• Provide a flexible schedule and work environment in various settings.
• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as required by state licensure and professional standards.
• Adhere to department and patient treatment schedules, complete timely clinical and billing documentation, and attend work-related meetings.
• Attend and contribute to patient care, staffing conferences, and other related meetings.
• Make referrals to other disciplines and agencies to address comprehensive patient needs.
• Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen.
• Adapt program and/or plan of care according to individual patient needs.
• Promote a safe environment and enforce the elimination of fire and safety hazards.
• Order supplies and equipment as necessary.
• Clean, maintain, identify, and report the need for repair of equipment.
• Prioritize patient service by ensuring the highest quality of care in a compassionate atmosphere, recognizing individual needs and rights.
• Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures.
• Synthesize information and interpret examination findings to establish a diagnosis, identify impairments, determine the predicted level of improvement and time required, identify precautions/contraindications, and design a plan of care.
• Develop appropriate treatment goals and methods in collaboration with patients and caregivers.
• Implement the physical therapy treatment plan and complete all related documentation and record keeping.
• Provide services including therapeutic exercise, functional training, manual therapy, airway clearance techniques, integumentary repair and protection, use of electrotherapeutic, physical agent and mechanical modalities, health and wellness education, chronic disease management education, falls assessment and interventions, and case management.
• Design, fabricate, fit, and/or apply adaptive devices and train patients in their use, including orthotics and prosthetics.
• Select appropriate activities suited to the individual’s physical and mental capacity, intelligence level, and interest.
• Instruct patients in skills and techniques for mobility activities and evaluate patient progress, attitudes, and behavior related to rehabilitative potential.
• Perform routine reexamination as needed/required to modify/progress the plan of treatment.
• Incorporate health and wellness programming into treatment plans.
• Perform home and community mobility assessments and provide recommendations for safe mobility.
• Perform other related duties as required.
