About The Position

VNA & Hospice of the Southwest Region is looking to hire a Physical Therapist (PT) for Franklin Country. Position can be full time or per diem. This home health Physical Therapist position earns a competitive wage and offers up to a $10,000 sign on bonus. Requirements

Bachelor's degree in Physical Therapy from an accredited university or college required.
Valid State Driver's License required.

Nice To Haves

Master's degree preferred.
2+ years of PT experience is preferred.

Can get the job done with minimal supervision.

Responsibilities

Evaluating, developing, implementing, and documenting a plan of treatment for patients of all ages.
Serving as a case manager providing supervision of students, physical therapist assistants (PTA), and home care aides.
Actively monitors and amends patient visit schedule to maximize productivity and patient need.
Performs a comprehensive (OASIS and PT) assessments of assigned patients.
Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.
Plans for and provides skilled physical therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc. assessing the patient's status and progress on each visit.
As Care Manager and/or team member, coordinates patient care with other clinical team members.
Supervises and directs clinical activities of PTAs, PT students and home care aides.
Completes and coordinates required paperwork.
