About The Position

VNA & Hospice of the Southwest Region is looking to hire a Physical Therapist (PT) for Franklin Country. Position can be full time or per diem. This home health Physical Therapist position earns a competitive wage and offers up to a $10,000 sign on bonus. Requirements

  • Bachelor's degree in Physical Therapy from an accredited university or college required.
  • Valid State Driver's License required.

Nice To Haves

  • Master's degree preferred.
  • 2+ years of PT experience is preferred.

  • Can balance multiple tasks at once while demonstrating good time management.

  • Can get the job done with minimal supervision.

Responsibilities

  • Evaluating, developing, implementing, and documenting a plan of treatment for patients of all ages.
  • Serving as a case manager providing supervision of students, physical therapist assistants (PTA), and home care aides.
  • Actively monitors and amends patient visit schedule to maximize productivity and patient need.
  • Performs a comprehensive (OASIS and PT) assessments of assigned patients.
  • Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.
  • Plans for and provides skilled physical therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc. assessing the patient's status and progress on each visit.
  • As Care Manager and/or team member, coordinates patient care with other clinical team members.
  • Supervises and directs clinical activities of PTAs, PT students and home care aides.
  • Completes and coordinates required paperwork.
