About The Position

Champion Rehab is seeking a compassionate, innovative, and energetic Physical Therapist to join their award-winning therapy team. This position offers the unique opportunity to gain experience and knowledge within an outpatient home care setting.

Requirements

  • Graduate of an accredited Physical Therapy program
  • Currently licensed to practice in State of MN. New graduates and temporary license holders encouraged to apply.

Responsibilities

  • Direct client care: continuous assessment of patient needs and functional deficits, patient care/treatment, on-going modifications to existing plan of care as needed, ability to initiate appropriate referrals to other disciplines when needed
  • Willingness to work closely with skilled and dedicated interdisciplinary team in order to maximize patient satisfaction and overall success based on diagnosis and comorbidities
  • Applying clinical expertise as necessary including but not limited to family and staff education/training, recommendations and guidance for safe and successful discharge, supervision of Physical Therapy Assistant, communicating with Physicians to ensure fluency of care
  • Documentation which follows regulatory guidelines (state, federal)
