About The Position

Join Main Line Health Home Care, the top-ranked home care agency in southeastern Pennsylvania, and make an impact with patients and their families within a Main Line area territory. As a Home Care Physical Therapist, you will be a key component in getting patients back up to speed by providing quality Home Care Physical Therapy services through comprehensive evaluation, program planning, and administration of treatment. You will perform detailed clinical assessments based on individual patient needs upon physician referral and develop a comprehensive plan-of-care based on findings following physical therapy evaluation, treating patients within the comfort of their own home.

Requirements

• 1 year clinical experience or Clinical Affiliation in Home Care.
• Graduate of a Physical Therapy program is required.
• Bachelor’s Degree required.
• Current Pennsylvania Physical Therapy license.
• Provider BLS (American Heart Assoc/American Red Cross) required on hire.
• Valid Driver’s license and insured automobile (as applicable).

Nice To Haves

• Master’s or Doctorate preferred.

Responsibilities

• Provide quality Home Care Physical Therapy services through comprehensive evaluation, program planning and administration of treatment to all patients.
• Perform detailed clinical assessments based on individual patient needs upon physician referral.
• Develop a comprehensive plan-of-care based on findings following physical therapy evaluation.
