About The Position

Innovate Rehab and Wellness is seeking a Licensed PRN Physical Therapist for a Skilled Nursing Facility. This is a PRN (as-needed) position with flexible, as-needed shifts designed to support work-life balance. The role emphasizes one-on-one patient care with less than 1% group/concurrent treatments. Requirements

• Doctorate or Master’s degree in Physical Therapy (accredited program)
• Valid state PT license
• Strong communication, problem-solving, and patient-care skills
• CPR certification preferred

Responsibilities

• Evaluate patients and identify movement and functional limitations
• Create and lead individualized treatment plans
• Help restore movement, function, and quality of life
• Work closely with an interdisciplinary team for optimal recovery
• Perform comprehensive PT evaluations
• Develop personalized, evidence-based treatment plans
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training
• Educate patients and caregivers on home programs and injury prevention
• Track and document progress; adjust plans as needed
• Collaborate with physicians, nurses, OTs, and other clinicians
• Stay current with PT best practices through continuing education
• Ensure compliance with all regulatory and ethical standards
