About The Position

Human Touch Home Health is seeking a Full-Time Physical Therapist (PT) to join our Blacksburg team serving patients throughout Blacksburg, Christiansburg, Radford, and surrounding New River Valley communities. This role offers more independence, one-on-one patient care, and a schedule that gets you out of the clinic and into the community.

Requirements

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical and communication skills
• Ability to work independently in patients' homes

Nice To Haves

• Home Health and OASIS experience preferred, but not required
• WellSky experience is a plus

Responsibilities

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Assess home safety and fall risks
• Educate patients and caregivers
• Monitor patient progress and adjust treatment plans
• Complete progress assessments and discharge planning
• Coordinate with physicians, nurses, PTAs, and other members of the care team
• Complete accurate and timely documentation using WellSky
