About The Position

UW Health in northern Illinois is seeking a full-time Physical Therapist for their Home Health program. This role involves providing outpatient physical therapy services, with a schedule of Monday-Friday 8:00 a.m. - 4:30 p.m., and every third Saturday from 8:00 a.m. - 4:30 p.m. The position is based at the UW Health 2550 Charles St Clinic. Additional compensation may include shift differentials for evenings, nights, and weekends, as well as overtime and on-call pay. Requirements

  • Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation).
  • Current license from the State of Illinois.
  • Required Basic Life Support/CPR Certification.
  • Valid State driver’s license and proof of liability insurance.
  • Must present acceptable evidence of licensure or authorization to test at the time of employment. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided.
