About The Position
Ultimate Therapy is seeking a licensed Physical Therapist to support rehabilitation services for adults in a long-term care and post-acute setting. The position centers on restoring function, preventing decline, and improving daily living skills.
Requirements
- Current PT license in good standing
- Ability to manage a varied caseload
- Team-oriented approach with strong interpersonal skills
Nice To Haves
- Prior experience in a rehab or SNF environment is a plus
Responsibilities
- Conduct initial and ongoing patient assessments
- Design goal-oriented treatment plans based on clinical findings
- Provide hands-on therapy to address strength, balance, and mobility
- Track outcomes and update care plans accordingly
- Instruct patients on proper techniques and home programs
- Collaborate with physicians, therapists, and facility staff
- Complete timely clinical documentation
