About The Position

Stern at Home Therapy is hiring Physical Therapists (PTs) to provide one-on-one care to patients in their homes in and around Newburgh, NY. They offer multiple employment structures including PRN, part-time, and full-time with various compensation models (W-2 per-visit, 1099 contractor, or salary). Full-time Physical Therapists under the 1099 tiered compensation model have the potential to earn up to $145,000 annually, based on visit volume. The role emphasizes treating one patient at a time, managing availability, and providing individualized care without the pressures of double-booking or productivity quotas common in traditional outpatient settings. The primary patient population is geriatric.

Requirements

• Active Physical Therapist (PT) license in New York required.
• Reliable transportation required.
• Strong clinical, communication, and time-management skills.
• Ability to work independently in a home-based setting.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one treatment based on each patient's functional needs and goals.
• Manage treatment plans with clinical autonomy.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, and members of the clinical team.
• Coordinate with the administrative team regarding scheduling, patient care, and logistics.
• Manage caseload based on availability and employment structure.
