About The Position

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time home health visit opportunity. This role involves servicing adult and geriatric clients in Atlantic County, NJ, providing rehabilitative care and skilled nursing to help them remain at home. The position emphasizes building meaningful relationships through one-on-one patient care, offering schedule flexibility, and focusing on quality care without clinic-based productivity pressures. You will work independently but be supported by a collaborative clinical team.

Requirements

A current NJ Physical Therapist - PT license.
A minimum of one year of recent work experience as a Physical Therapist - PT.
Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem solving skills.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Nice To Haves

One year prior clinical experience as a licensed Physical Therapist is preferred but not required.

Responsibilities

Perform home visits in designated geographic territories.
Complete comprehensive physical therapy evaluations.
Assess mobility, strength, balance, gait, transfers, endurance, and safety risks.
Develop individualized treatment plans to improve function and independence.
Provide therapeutic exercise, gait training, balance training, and other evidence-based interventions.
Collaborate with nurses and other clinicians as part of a multidisciplinary care team.
Educate patients and caregivers on exercises, safety, and disease management.
Document patient care using a user-friendly electronic medical record system.
