About The Position
Symbii Health, a Pennant Group Company, is seeking a compassionate and skilled Physical Therapist (PT) to join their home health team in Rock Springs, WY. This role involves providing one-on-one, patient-centered care in patients' homes to help them regain independence and improve their quality of life. Requirements
- Doctorate or Master’s degree in Physical Therapy
- Current PT license in the state of Wyoming (or ability to obtain)
- Strong clinical, communication, and organizational skills
- Valid driver’s license and reliable transportation
Nice To Haves
- Home health experience preferred, but not required
Responsibilities
- Evaluate and develop individualized care plans for home health patients
- Deliver evidence-based physical therapy interventions
- Educate patients and caregivers on treatment plans and progress
- Collaborate with an interdisciplinary team to ensure optimal patient outcomes
- Maintain accurate and timely clinical documentation
- Adapt care based on patient progress and needs
