About The Position

Therapy Management Corporation (TMC) is hiring a Physical Therapist to provide patient-centered therapy to adults and older adults. The role focuses on helping patients improve independence, mobility, safety, and quality of life through personalized treatment and ongoing assessment of progress. The Physical Therapist works in a collaborative clinical environment that emphasizes quality outcomes, consistent care, clinical autonomy, and rehabilitation rather than high patient volume. TMC partners with skilled nursing facilities and senior living communities to provide rehabilitation services.

Requirements

Licensed Physical Therapist (PT) or eligible for licensure in the state of practice.
Graduate of an accredited Physical Therapist program.
Passion for patient-centered care and improving quality of life for older adults.

Interest or experience in skilled nursing, long-term care, or geriatric rehabilitation.

Responsibilities

Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance, and functional independence.
Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals.
Deliver hands-on therapy interventions to support recovery, increase endurance, and improve quality of life in older adults.
Help patients restore functional mobility and safely perform daily activities with greater confidence and independence.
Partner with interdisciplinary teams, caregivers, and nursing staff to support patient outcomes and continuity of care.
Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals.
