About The Position
We are seeking a skilled and compassionate Physical Therapist (PT) to join our team on a PRN basis, serving patients in the Lakeway and Bee Cave areas. In this role, you will provide high-quality, patient-centered care in the home health setting, helping individuals regain mobility, manage pain, and improve overall functional independence.
Requirements
- Graduate of an accredited Physical Therapy program
- Current, active Physical Therapist license
- Current CPR certification
- Valid driver’s license and reliable transportation
- Current auto insurance
Nice To Haves
- Minimum of 1 year of home health experience preferred
- Experience with HomeCare HomeBase (HCHB) preferred
Responsibilities
- Perform and document comprehensive initial evaluations to assess patient needs, identify functional limitations, and establish a clinical diagnosis prior to intervention
- Develop and implement individualized treatment plans designed to restore function, improve mobility, relieve pain, and prevent disability
- Regularly evaluate patient progress and modify treatment plans to ensure optimal outcomes
- Apply infection control and universal precaution standards in all patient care activities
- Ensure continuity and quality of care through accurate, timely, and compliant documentation
- Communicate effectively with patients, families, interdisciplinary team members, and physicians
- Support and uphold the organization’s mission, values, and clinical standards
- Complete all required documentation within established company timelines and guidelines
