About The Position

Physical Therapists at Dartmouth Health Home Care (DHHC) have the unique opportunity to truly get to know their patients, becoming an integral part of their care team. With an average visit of 45 minutes, you'll have significant time to provide meaningful therapy. Requirements

• Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school in Physical Therapy.
• Unencumbered PT license in VT and NH or compact.
• Driver’s license, minimum requirements on car insurance, and a clean driving record.

Nice To Haves

• One year of experience as a PT in a home health care setting is preferred.

Responsibilities

• Screen, evaluate, re-evaluate, and treat patients.
• Coordinate and communicate treatment activities with interdisciplinary team members and the patient.
• Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.
• Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.
• Communicate with all members of the health care team.
• Document evaluations, treatment goals, and objectives.
• Teach patients and family members the rationale for treatments and procedures.
